A UPS MD-11 cargo jet erupts in flames after a catastrophic engine separation during takeoff from Louisville Muhammad Ali International Airport. In this in-depth analysis, Jeff Ostroff breaks down exactly how an engine can fall off an aircraft, what may have gone wrong with this jet’s pylon mounts, and why NTSB investigators are now looking closely at maintenance records, structural fatigue, and design legacy from the DC-10 era.
You’ll see aerial footage, 3D diagrams, and a clear explanation of the takeoff sequence — including what V1, Vr, and V2 really mean for a heavy aircraft like this. Then we compare this tragedy to the historic American Airlines Flight 191 disaster in 1979 to show what’s changed — and what hasn’t — in nearly five decades of aviation engineering.

Yeah, let's put a petroleum plant at the end of a runway. I'm sure nothing bad could happen. Duh!
This was right outside my house. We couldn’t believe it. Our phones kept doing the amber alert tone, and we didn’t really know what was happening. We just saw a huge amount of fire and smoke.
Naturally we went to our basement just in case because we weren’t sure if it was a chemical explosion or something.
Horrible this happened. RIP to everyone involved.
HOW THE HECK ARE YOU GONNA HAVE A RUNWAY POINTED STRAIGHT AT BUILDINGS TOWARDS A PETROLEUM RECYCLING PLANT, THAT'S JUST PURE STUPIDITY JUST A PERFECT STORM WAITING FOR A BIG ASS EXPLOSION TO HAPPEN IF A PLANE CRASHED AND LOOK IT HAPPENED WHO'S GENIUS IDEA WAS IT TO EVER BUILD THAT RUNWAY AND RECYCLING PLANT ALL THAT STUFF LIKE THAT NEXT TO EACH OTHER BUT THE RUNWAY POINTING STRAIGHT AT IT IS CRAZY…
Antique aircraft!
The only two employees left at the NTSB since trump fired everyone else
My heart goes out to the people on the plane and there families, my God bless and keep them 🙏 ❤
I'm sure it has nothing to do with tRump reducing regulations and staffing.
if a plane is crashing do they allways blow up? wouldnt it make survival better % if the plane was able to dump its fuel tank safely before crashing?
Trump's America.
ST Engineering is shaking in its boots
